Test selection depends on whether current infection must be shown. Breath and stool antigen tests, or biopsy-based tests during endoscopy, reflect active infection; serology reflects exposure and is not suited to confirming eradication.

H. pylori blood antibody tests cannot distinguish active infection from past infection, so they are not commonly used for diagnosis.
